Eligibility Criteria

  • Experiencing a behavioral health crisis within the past 72 hours or transitioning from or at risk of requiring a higher level of care.
  • Requiring short-term assistance to identify and engage in necessary services that promote community safety and stability.
  • Displaying moderate to high behavioral or emotional challenges that, without intervention, may disrupt functioning in areas such as family, housing, education, social interactions, employment, or community involvement.

Community Stabilization Services

Community Stabilization provides short-term support for individuals transitioning between levels of care when immediate services are unavailable. This service includes brief therapeutic interventions, skill-building, crisis de-escalation strategies, and follow-up care coordination to promote long-term stability.

Designed as a bridge between crisis response and ongoing care, it ensures individuals receive essential resources while minimizing the need for higher levels of intervention.
